RELEASE OF LIABILITY

KRAFTY SHACK RELEASE OF LIABILITY LEGAL AGREEMENT

 

In consideration of Krafty Shack, when registering my child for ANY ACTIVITY AT OUR LOCATION,  I acknowledge, accept and agree to following policies, waiver & release of liability:

 

I understand that I am leaving my child at my own will and risk and accept full responsibility for any liability while my child is attending ANY ACTIVITY AT OUR LOCATION.  NO EXCEPTIONS.
 

I authorize Krafty Shack to act for me in case of any emergency and in need of medical attention.
 

I understand that I have to pick up my child on time at the end of the camp or class.
 

I understand that some photos may be used on our website or social media. 

 

I understand that my child shall be removed from the camp due to unacceptable conduct without a refund. 

 

I understand that Krafty Shack is not a day care facility and that is not licensed by  Texas Health and Human Services.

By signing this contract, I AGREE AND DECLARE KRAFTY SHACK, OWNERS AND STAFF ABSOLUTELY FREE OF ANY LIABILITY AND WAVE ANY AND ALL RIGHTS TO SUE.
